- Name of the person requesting
- Shopee username of the person requesting
- Shopee username of the account to be terminated
- Relationship with account owner
- Proof of Relationship
- Front and back copies of the Identification Card (Requestor)
- Front and back copies of the identification Card (Account Owner)
- Death Certificate
- Wasiat/Faraid Certificate (For Muslims)
- Grant of Probate/Will (For Non-Muslims)
- Letter of Administration
